By accessing our website, booking appointments, enrolling in digital training programs, or engaging in any services provided by Unlimited Rehab & Performance (“company,”, “we.” “us,” or “our”), you agree to be bound by these Terms of Service.

2. Services Provided
Unlimited Physical Therapy & Sports Performance provides:
Physical therapy services
Dry Needling
Performance training
Movement and strength assessments
Digital training and coaching programs
All services are provided in North Carolina and are governed by North Carolina State laws
3.Client Responsibilities
The client agrees to:
Provide accurate health and personal information
Follow treatment recommendations
Use digital home exercise program responsibility
Maintain updated payment information
Obtain guardian consent for minors
4.Health Information and HIPAA Compliance
All health information collected in the course of care is protected under HIPAA and applicable state law.
Clients authorize the Company to collect, store, and use PHI for treatment, payment, and health care operations.
5. Payment, Billing, and Auto-Renewal
5.1 Payment Terms
All payments for physical therapy and online programs are due at the time of purchase. Payment information is processed securely through approved third-party processors.
5.2 Refund Policy
The owner of Unlimited Physical Therapy & Sports Performance, Megan Rodio, reserves the right to give refunds on a case by case basis. This typically involves a patient who is no longer able to safely come to physical therapy, a change in presentation where physical therapy is no longer appropriate, patient is moving, etc.
6.Minors
For clients under the age of 18:
A parent or legal guardian must provide consent for treatment and data collection
Guardians are responsible for payment and compliance with these terms.
7.Exercise and Training Risks
Participation in physical therapy and performance training carries inherent risks. By engaging in services, clients acknowledge:
Physical activity may involve injury risks
They assume all risks associated with participation
They must disclose changes in health status promptly
They may be required to obtain physician clearance
8.Limitation of Liability
To the fullest extent permitted by law, Unlimited Physical Therapy & Sports Performance is not liable for:
Indirect, incidental, or consequential damages
Loss of data
Business interruption
Injuries resulting from misuse of training programs or failure to follow instructions
9.Intellectual Property
The website content, online programs, and documentation are the intellectual property of Unlimited Physical Therapy & Sports Performance.
Unauthorized reproduction, distribution, or modification is prohibited.
10.Termination of Services
We reserve the right to refuse or terminate services for:
Repeated no-shows
Failure to pay
Unsafe behavior
Violation of these terms
